Job Description

Through experience, skill and knowledge of brain injury, spinal cord injury, cerebral palsy and orthopaedic injury, my client empowers individuals to adapt and develop a meaningful lifestyle after a devastating traumatic injury. As a company they use every specialised resource available to assist people disabled by traumatic injuries to maximise their potential and live a fulfilling life again. They focus on putting meaning back into living at all times and so far have positively supported over 700 clients and their families adjust, find purpose and enjoy life again.

Due to continued growth they are now looking for a Case Manager to work in the West Midlands area, your base will be from home and you will need to have your own transport (mileage and expenses paid), you'll also be happy to travel in around the area. 

Previous experience would be an advantage, however we would also look at applications from other areas such as Nursing, Paediatrics, Speech Therapy, Physiotherapy.

Duties will include: 

Client Care

Undertake a full needs assessment including risk assessment in order to develop a personal therapeutic programme and a quality support package for the client in line with need.

Develop and maintain a communications framework so that the client, family and friends are central to the ongoing process.

Develop specific goals to meet the needs of the client in order to help them make gains in their physical cognitive and psychological well-being.

Monitor and evaluate goals so they are readdressed and updated in line with client need.

Liaise with professionals so that there is a clearly joined up programme involving all relevant parties to achieve a high quality treatment programme.

Documentation

To keep accurate and contemporaneous records.

To prepare case plans, risk assessments, support plans, progress reports and/or needs assessments, and review reports in line with policies and procedures.

Recruitment and Training

To work with the Assistant Case Manager to recruit Team Leaders and/or Support Workers to provide care in line with the therapeutic plan.

To undertake the induction of new Assistant Case Managers, and to continue to monitor and supervise through their probationary period.
